OSHA Form 300A Log Summary Must be Posted by Feb. 1, 2017

Where can I find Injury & Illness Recordkeeping forms 300, 300A, 301?

When must you post the summary?
Employers must post the Summary only, not the log, by February 1 of the year following the year covered by the form and keep it posted until April 30 of that year.

How long must you keep the Log and Summary on file?
Employers must keep the Log and Summary for 5 years following the year to which they pertain.

Do you have to send these forms to OSHA at the end of the year?
No. You do not have to send the completed forms to OSHA unless specifically asked to do so.
Note: As of July 1, employers will be required to submit electronically depending upon their establishment size and industry. (See graph below)

I have less than 10 employees, do I need to complete and post?
No. Employers with 10 or fewer employees throughout the previous calendar year do not need to complete these forms.

Compliance Schedule

The new reporting requirements will be phased in over two years:

*Beginning in 2019 and every year thereafter, the information must be submitted by March 2. *Establishments with 20-249 employees are limited to certain high-risk industries.
If you, as an employer, do not fall in the either of the above two categories, you are only required to electronically submit data when requested by OSHA. Annual documentation is still required unless otherwise exempt from the recordkeeping requirement.
